College of Health Science Immunization Policy

Immunizations Required

  1. Tdap: Tetanus, Diphtheria, and PertussisSingle dose Tdap on or after age 10 and a booster dose every 10 years thereafter.
  2. MMR: Measles, Mumps, and Rubella2 doses are required. MMR positive titer or 2 vaccination dates
  3. Varicella positive titer or 2 vaccination dates
  4. Tuberculosis (TB) Testing: QuantiFERON blood test within the past 12 months. If there is a history of positive PPD, provide a negative Chest X-Ray (valid for 1 year) BOTH documentation of positive test result and negative chest x-ray are required.
  5. Annual Influenza Vaccine during current respiratory season.  Students who start in the Spring and have not yet received the current seasonal vaccine must obtain it prior to the start of the program.
